2、有HCV感染者性别、年龄、长期饮酒、长期吸烟、肝功(AST、ALT水平)、HCVRNA、糖尿病、脂肪肝、是否抗病毒等因素发展至肝硬化、原发性肝癌的影响进行回顾性分析。结果经单因素分析,病例组与对照组在性别、年龄、AST、ALT水平、HCVRNA、脂肪肝无相关(p>0.05)。单因素分析可能与丙肝相关性肝硬化、原发性肝癌有关的因素有长期饮酒史、长期吸烟史、糖尿病、未行抗病毒治疗(p<0.01)。多因素非条件Logistic回归分析,最终上述4个因素均进入回归模型,包括长期饮酒史、长期吸烟史、糖尿病、未行抗病毒治疗。且标准回归系数在肝硬化组分别为0.423、2.932、2.72
3、4、1.397;在原发性肝癌组分别为0.425、2.937、2.729、1.399,且p<0.05。经过多因素回归分析,上述4项指标全部有意义。提示长期饮酒史、长期吸烟史、糖尿病、未行抗病毒治疗为HCV感染进展为肝硬化、原发性肝癌的独立危险因素。结论慢性丙型肝炎患者若同时伴有糖尿病、长期吸烟史、长期饮酒史、未行抗病毒治疗等因素,都是肝硬化、原发性肝癌的高危人群,而与患者的性别、年龄、AST、ALT水平、HCVRNA、脂肪肝无明显相关。临床需予以注意及预防这些高危因素,防止肝硬化、原发性肝癌的发生。硕士研究生:朱桂晓(传染病学)指导老师:边城(教授)【关键词】丙型肝炎病毒;肝
