1、磁共振弥散加权成像和增强扫描在腹膜转移癌中的应用价值ABSTRACTObjective：Toexploretheefficacyofconventionalmagneticresonanceimaging(MRI),diffusion-weightedmagneticresonanceimaging(DWI),andenhancedmagneticresonanceimagingindiagnosingperitonealcarcinomatosis(PC),andsummarizetheMRIfindingsofPC.Methods:FromSeptemberin20
2、13toMarchin2015,43patientswithperitonealcarcinomatosiswereincluded,whichconfirmedbysurgicalpathologyorclinicalfollow-upinGansucancerhospital.AllpatientsunderwentMRexamination(includingT1WI,T2WI,DWIandenhancedmagneticresonanceimaging).Allofsequenceswerereviewedbytworadiologistsindependent
3、lywhowereblindedtotheresultsofthepathologyandclinicalfollow-up.Thefollowingsubjectswereanalyzedandrecorded:(1)thelesiondistributionandtheMRIfindings;(2)thesensitivity,specificity,positivepredictivevalueandnegativepredictiveofcommonMRsequences,commonMR+DWI,commonMR+DWI+enhancedindiagnosin
4、gPCwerecalculated.Andthecorrespondingreceiveroperatorcharacteristic(ROC)curveweredrawnbySPSS20.0.Results:1)Totalof28patientswithperitonealcarcinomatosiswereenrolled.TheMRfindingsofPCwereasfollowing:irregularlinearthinckeingofparietalandvisceralperitoneum(20),serosalthinckeingoftheintesti
5、nalsurface(2),omentalstrandingandcaking(10);mesenterystrandingthickening(9);massesandnodulesintheabdnominalcavitys(22);ascites(26);lymphadenopathy(8).Thetotalnumberoflesionlocationswas240,withthecommonsitesofbilateraldiaphragm,bilateralparacolicsulci(rightmorethanleft),omental,smalloment
6、um,peritoneum,andpotwallofuterine/bladder-rectalpouch.2)TheKappavalueofthethreemethodsbytworadiologistswere0.852、0.850、0.846respectively.Theyachievedgoodconsistency.Thesensitivityandspecificityvalueofthethreemodesestimatedbyradiologist.Awere80.0%and55.6%,85.7%and73.3%,92.9%and80.0%respec
7、tively,andtheAUCvaluewere0.690(P=0.041),0.795(P=0.002)and0.864(P=0.000),respectively.The95%ofconfidenceintervalintheAUCvaluesofthethreescanningmodeshasoverlapped,indicatingthatthereisnostatisticallysignificantdifference(P>0.05)amongthethreescanningmodes.ReviewerBachievedr
