2、MJ)internalderangementwhohaveundergoneTMJdiscanchorageformorethan5years,toacquirethelong-termresultsoftheoperationandtoanalyzetheinfluenceofage,IDstageandotherfactorsontheefficacyofoperation,tomakeapreliminaryanalysisofthecausesofrecurrence.METHODS:61patients(

3、76joints)wereincluded.AllthecasesunderwentTMJdiscanchorageintheDepartmentoforalandmaxillofacialsurgery,ninthpeople’sHospital,affiliatedtoShanghaiJiaoTongUniversitySchoolofMedicineduringFebruary2004toJune2008.Theywereallfollowedupformorethan5years.Clinicalsurve

4、ysweretakenfromallthepatientstocompareclinicalsymptomsandmandibularmovementbeforeandafteroperation.MRIscansweretakentoevaluatepostoperativediscpositon.IDstage,disclengthandcondyleheightweremeasuredonMRI.Thepatientswereclassifiedbyage,IDstage.Thedatawerestatist

5、icallyanalyzedandcomparedbetweendifferentgroup.V万方数据上海交通大学医学院硕士学位论文RESULTS:Theaverageageofpatientswas39.85±12.76yearsold(14~64)andthemeanfollow-uptimewas71.34months(60~112months).Theaveragemaximummouthopeningimproved14.34±5.87mmcomparedwithpreoperativeexaminat

6、ionandthemeanvalueofVisualanalogscale(VAS)decreasedby33.44±20.56.Thereweresignificantdifferencesinbothresult(p<0.05).Theefficiencyofclinicalevaluationwas84.21%(64/76)andthepatients’satisfactionratewas85.53%(66/76).Follow-upMRIscansshowedthatofall76joints,68cas

7、eswerejudgedasexcellentandgoodondisc-condyleposition(89.67%).TheFisher’sanalysisamongthreeevaluationresultsshowednodifference(p>0.05).OfallthepatientswhowereevaluatedaspooronMRIthecondylarheightdecreasedbyanaverageof0.43±1.36mm,whileitincreasedbyanaverageof0.6

8、7±1.88mmwithexcellentrelationshipandanaverageof0.51±1.09mmwithgoodrelationship.Therewasstatisticaldifferenceofcondylarheightbeforeandafteroperation(p<0.05).5jointsshowedobviousposi
