7、·····················40中文摘要右室不同部位起搏对患者心功能及情绪状态的影响摘要目的:自从心脏起搏技术应用于临床以来,挽救了一大批病态窦房结综合征及高度房室传导阻滞患者的生命,并成为缓慢性心律失常唯一有效的治疗方法。多年来,由于右室心尖部肌小梁排列密集、电极植入操作相对简单等特点,右室心尖部电极植入技术已被广泛应用于临床之中。随着起搏器植入适应症的不断完善及植入患者的不断增加,近年来研究发现,右室心尖部起搏会引起左右心室电激动传导差异,进而产生机械运动失同步,导致血流动力学障碍及心脏灌注受损,严重影响患者

8、的生活质量,增加了起搏器植入患者由于心衰、心律失常等疾病的住院率及死亡率。随着主动电极的出现,右心室间隔部位起搏应运而生,右室间隔部起搏时,心室最早激动点更靠近希氏束,起搏电极发放起搏脉冲,使电激动从室间隔向双心室和心尖部扩散,完成接近生理状态的激动传导,因而能够改善心室间电
